C1500 2WD with the heavy-duty brake package (RPO JB5, 1.25" vented rotor). Standard on all 1992-1998 trucks regardless of cab style, and on 1988-1991 extended cabs. If you have a 1988-1991 regular cab, check your build sheet, most ran the lighter standard-duty brakes instead.
